Thank you Steve,
I solved the first issue.
The pdf file is still give an error, even in the file list there was
application/pdf
application/x-pdf
I added also
image/x-pdf
text/pdf
adobe/pdf - doesnot worked.
I was not able to upload pdf file also to your live demo
I will right this issue to a new topic on forum.
