Hi,
When I try and add a file, by doing the following:
1. Log-in as admin
2. Set the File Location to a word document
3. Set the category to Presentation
4. Set Department to Information Systems
5. Set Authority to read
6. type stuff in description and comment, and leave permissions settings alone.
I then click Add Document, the screen goes blank and then the Add Document form re-appears again emptied of information.
I am using php v4.1.2, Apache, and internet explorer v6.
Anyone seen any similar problems?

I have not seen this before. Of course, I use firefox/linux.
